If you are eligible for GP Management Plan and Team Care Arrangement (TCA), your GP can give you a referral under Chronic Disease Management (CDM) system which allows you to have subsidised appointments under Medicare. For this appointment, you are required to pay our standard fee and our admin staff will assist you with submitting a Medicare rebate claim. Eligible patients can access Medicare rebate for up to 5 visits (in total) per calendar year.

Note: These rebates are shared with other Allied Health Providers (such as Dietitian, Diabetes Educator, Exercise Physiologist, Physiotherapist, Chiropractor, Occupational Therapist, etc...).
